Use your FMEA or defect data.<\/li>\n\n\n<li><strong>Find where it starts.<\/strong> Go to the step where the mistake is made, not where it is found.<\/li>\n\n\n<li><strong>Ask: can we prevent it?.<\/strong> Shape, sequence, system rule. Make the wrong way impossible.<\/li>\n\n\n<li><strong>If not: detect it at once.<\/strong> Sensor, count, colour, alarm.
